Below are registration rates for the NCPN 2013 conference being held at the Grand Hyatt San Antonio.

ONLINE REGISTRATION
Please complete and submit the entire ONLINE registration form link below to speed your registration. (One form per participant. Resubmit the form for additional registrants.) Incomplete forms cannot be processed. All forms must include payment or purchase order. Confirmations will be sent to the registrant. Invoices will be sent to the billing organization.

CANCELLATIONS / SUBSTITUTIONS
For individuals who must cancel their registrations, the following rules apply:
- All cancellations must be in writing.
- Cancellations received by September 14 will be charged a $50 cancellation fee.
- Cancellations received between September 15 and October 4 will be charged 50% of the total registration fee.
- Cancellations are received after October 4 and conference no-shows will be responsible for the full registration fee.
- Those who determine they cannot attend the conference after they have registered may substitute colleagues in their place. Please email llocke@cord.org for further details on submitting a substitution request.
